Some courses include pre-course assignments so early registration is encouraged, we recommend that you register before 12/10/05. Registrants are required to select a credit option when registering. Please complete the registration form noting your credit choice. The following credit options are available for this Institute:

Continuing Education Units
Continuing education credits in the form of a CEU (Continuing Education Unit) certificate is available for all Institute courses. Participants must meet course requirements specified by instructors to be eligible for this certificate.

Graduate Level Academic Credit
University of Florida or University of Minnesota graduate level academic credit is available to qualified students. These course credits may be applicable to a degree program in public health or in another field. It is the responsibility of the participant to determine the transferability of these credits to their academic program or institution.

Prerequistes for taking graduate level academic credit: Participants must be a graduate or professional degree student or receive consent from the course instructor. Requests for consent will be handled through CPHEO. Those registering for graduate level academic credit must choose from the following categories:

Degree-Seeking Students are those students who have been formally admitted and are currently enrolled in a degree program at the University of Florida or at the University of Minnesota Twin Cities Campus.

Non-Degree Seeking Students are those students needing graduate level course work that appears on a graduate career transcript but who have never attended the University of Florida or the University of Minnesota Twin Cities Campus or have attended but are no longer active in a degree-seeking program.

Registration for academic graduate credit will be processed through the respective Student Services Office at each University determined by course designator and number. Tuition and fees paid will be credited to individual student accounts by CPHEO, University of Minnesota.

Specialty Credit Information

ALL SCHOLARSHIPS HAVE BEEN AWARDED AT THIS TIME.

Cancellation Policy
100% of tuition and fees will be refunded if written notification is received by January 2, 2006.
50% of tuition and fees will be refunded if written notification is received by January 3, 2006.
No refund is available after January 4, 2006.

Course Location
Institute classes will be held in the new Health Professions, Nursing and Pharmacy Building (HPNP) on the campus of the University of Florida, Gainesville, Florida.

Specific directions and maps with campus parking information will be sent to participants upon receipt of their completed registration form.
